Effective Monday, August 3, 2009, the new Employee Wellness Clinic is at Courtyard Primary Care.
Where is the clinic located?
Courtyard Primary Care is located at 4410 N. Midkiff, Suite D-6, in the shopping center across from the mall. The entrance to the clinic faces Taco Villa.
Who are the providers?
The providers at Courtyard are Donald Hillman, MPAS, PA-C, and Tiffany Locke, MMS, PA-C.
What services are offered?
Services include primary care and minor emergency care with a focus on wellness and prevention. Courtyard Primary Care welcomes patients of all ages, from infants to geriatrics.
Who is eligible?
All City of Midland employees covered under our group health plan, both active and retired, as well as their covered dependants. There is no charge for medical services received by employees and dependants enrolled in the group health plan.
