This is the official form for the City of Midland. It is the only form that will be accepted and any forms submitted in a different format will be returned.
This section of the site is for organizations that may be required to have a backflow device installed/inspected:
- If you have your backflow device inspected or installed please keep a copy of this for your records. It is the plumber’s responsibility to submit this form to the City of Midland but the facility should reserve a copy in case the inspection is questioned.
- The City of Midland recognizes the International Plumbing Code which requires every backflow device to be inspected annually. The City of Midland is currently allowing irrigation and any non-health hazard applications that require a Double Check Valve Assembly (DCVA) to be inspected every other year. (* irrigation units that directly apply chemicals will be required to get an inspection yearly and the type of unit needed is a Reduced Pressure Principle backflow device.)

This section of the site is for Backflow Prevention Installers and Inspectors:
- The City of Midland is currently separating all Backflow Prevention forms between the Utilities Department and Code Administration. Code Administration receives all forms that pertain to devices contained inside the facility and sprinkler systems. Utilities Administration receives all forms pertaining to devices directly between the facility and the meter. Please see the PDF below for diagramed instructions.
