  • ALL SERVICES WILL BE BY APPOINTMENT ONLY.

  • MASKS WILL BE REQUIRED AT ALL TIMES WHILE INSIDE FOR ALL ADULTS AND CHILDREN OVER THE AGE OF TWO (2) YEARS OF AGE.

  • ONLY THE PERSON RECEIVING SERVICES AND 0NE (1) GUARDIAN, IF THE PATIENT IS A MINOR CHILD, WILL BE ALLOWED INSIDE

Please bring the following to your immunization appointment

  • Picture Identification

  • Immunization Records for every one receiving immunizations

  • Insurance or Medicaid Card
