The Community Healthcare Program (CHP) is a collaboration between the Midland Fire Department Emergency Medical Services, Midland Memorial Hospital, and community-based health and social service agencies. It provides scheduled, individualized care to high-risk patients in their homes. CHP aims to deliver the best care to Midland's citizens and continuously improves its services and focus on quality care.
